Summary of Client and Program Goals
Describe your client briefly. Summarize the client goals, as well as any additional program goals you have established for the client. If you are using exercise to improve health, you should cite your source for why exercise will help the client’s health. Notice the Level 1 heading (above) which is bold and centered, but still in Times New Roman 12 point font.
Client Health and Safety Concerns
This section should include the ACSM risk stratification classification and information about how you established this stratification level. Clearly outline any known disease or signs and symptoms of disease according to the ASCM risk stratification process, as well as any positive or negative risk factors.
Guidelines for the Initial Four Weeks
Give a brief overview or introduction to the client plan for the first four weeks of the exercise program. Each of the following sections should be approximately half a page in length. Throughout the remainder of this assignment, in-text citations should be used whenever recommendations from a textbook or other source are followed.
Intensity
Explain the intensity you have selected for your client through the first four weeks and why you selected this intensity. Be sure to include the intensity of the warm-up, conditioning phase, and cool-down. Intensity should be determined by one of the methods discussed in class. Note the Level 2 heading which is bold, but left aligned.
Duration
Explain the duration or time (in minutes) you have selected for your client through the first four weeks and why you selected this duration. Be sure to include the duration of the warm-up, conditioning phase, and cool-down.
Frequency
Explain the frequency you have selected for your client through the first four weeks and why you selected this frequency.
Type
Explain the type of cardio equipment you have selected for your client through the first four weeks and why you selected this/these machine(s).
Progression
Describe the progression you have designed into the first four weeks of this exercise program, if appropriate. Detail which components of the F.I.T.T. principle will change and why, including in-text citations as needed.
Other Considerations
Describe any other guidelines or considerations involved in the initial four weeks. This should include an appropriate warm-up, cool-down and stretching information. PLEASE BE VERY BRIEF IN YOUR DESCRIPTIONS OF THESE ACTIVITIES. (For example, the client would stretch statically for 10 minutes after the conditioning phase of the workout, or the client will warm-up on the treadmill). This section should be no more than half a page.
Conclusion
A conclusion statement should be included to wrap up this assignment. This should be 2-3 sentences summarizing the program and expected benefits from the program.
